Apple Watch HRV: how it is measured and how accurate it is
Apple Watch measures heart rate variability automatically in the background, at moments when you are still, and stores it in Apple Health as SDNN in milliseconds. Apple's HealthKit documentation states that it "uses SDNN heart rate variability." Accuracy is good for beat timing at rest, but one study found HRV values that differed from a chest strap by about 20 ms on average. Use it for your own trend, not for comparison with other devices.

How does Apple Watch measure HRV?
Apple Watch measures HRV with its optical heart sensor, which detects each pulse at the wrist. When you are still, it records the time between beats and calculates SDNN, the standard deviation of the intervals between normal heartbeats. Apple's documentation says "the system automatically records samples on Apple Watch," and its support page says HRV "is sampled while you're still."

On older models, the watch takes relatively few readings, so a day's value can rest on a handful of samples taken at different times. The Series 12 page says HRV is "now included in your overnight and new daytime vitals."
What is new with Apple Watch HRV in 2026?
In 2026, Apple added more frequent HRV readings and a Recovery HRV view on its newest watches. Apple says Series 12 measures HRV "as often as every five minutes" and includes it in overnight and daytime vitals and a new readiness score. Apple's developer documentation also lists an RMSSD data type for iOS 27, next to the existing SDNN type.
What Apple states on its own pages:
- Recovery HRV: "If you have Apple Watch Series 12 or Apple Watch Ultra 4, you can view your Recovery HRV." Apple describes it as measuring "how time varies between heartbeats," and says it "can be an indicator of stress and recovery."
- Readiness: a score "determined each morning by analyzing your recent activity, vitals, and sleep."
- Requirements: Series 12, Ultra 4 and SE 3 require iPhone 11 or later with iOS 27 or later.
What Apple does not state on those pages: the formula behind Recovery HRV. Do not assume it is RMSSD, because Apple's support page does not say so. If an app shows your HRV, check whether it reads the SDNN or the RMSSD data type. For the difference, see RMSSD vs SDNN.
How do you check HRV on Apple Watch?
You check Apple Watch HRV in the Health app on iPhone. Tap Browse, then Heart, then Heart Rate Variability, or search for it. The chart shows day, week, month and year views. On Series 12 and Ultra 4, Apple says you can also open the Heart Rate app and turn the Digital Crown to see Recovery HRV.
Steps:
- Open the Health app on iPhone.
- Tap Browse, then Heart.
- Tap Heart Rate Variability.
- Switch to W (week) or M (month) to see the trend.
- Tap Show All Data to see individual readings with their times.
To prompt a reading, many people sit still and start a Breathe session in the Mindfulness app. A 2025 validation study timed its Apple Watch HRV recordings to Breathe sessions (Bonneval et al., 2025). If you do this, do it at the same time and in the same position each day.
How accurate is Apple Watch HRV?
Apple Watch is accurate for heartbeat timing at rest, but its HRV values differ more from reference devices. Against a Polar H10 chest strap with analysis software, Series 9 and Ultra 2 underestimated HRV by 8.31 ms on average, with a mean absolute error of 20.46 ms (O'Grady et al., 2024). Resting heart rate was much closer.

The pattern across studies: the watch captures heartbeats well at rest, but HRV, which depends on small differences between beats, is more sensitive to missing beats and filtering. O'Grady and colleagues conclude that their findings "underscore the need for improved HRV algorithms in consumer wearables and caution in interpreting HRV data."
Accuracy is best when you are still. Bonneval et al. found the highest agreement during rest, compared with talking, watching a movie or walking. None of these studies tested people with irregular heart rhythms.
Is Apple Watch HRV useful for tracking recovery?
Apple Watch HRV is useful for tracking your own trend, especially as a weekly average next to resting heart rate and sleep. It is less useful as an exact number to compare with other people, research tables or other devices. Consistent wear, especially overnight, gives more readings and a steadier trend.
Research shows the trend carries information. In health care workers wearing an Apple Watch, the daily pattern of SDNN differed in the 7 days before and after a COVID-19 diagnosis compared with healthy periods (Hirten et al., 2021). That is a group finding, not a diagnostic test. If you feel unwell, see a doctor.
Tips for a cleaner trend:
- Wear the watch overnight, snug but comfortable.
- Compare weekly averages, not single readings.
- Read HRV together with resting heart rate. Both moving in the wrong direction is a stronger signal. See resting heart rate by age.
- Do not mix Apple Watch SDNN with RMSSD from another app or device.
Why is Apple Watch HRV lower than Oura or Whoop?
Apple Watch HRV often looks lower than numbers from Oura or Whoop because Apple Health stores SDNN from short readings, while many other wearables report RMSSD averaged over sleep. Different formulas, windows and times give different values for the same person. A lower Apple Watch number does not mean lower recovery.
